Form 2
  1. Employee registration form with SOCSO to be submitted by employers whenever there is a new employee with no prior SOCSO account. 
  2. Applicable for Malaysians and permanent residents.

Foreign Worker's Registration Form
  1. Employee registration form with SOCSO to be submitted by employers whenever there is a new employee with no prior SOCSO account. 
  2. Applicable to foreign employees. 

Form 8A
  1. SOCSO contribution statement to be submitted by employers to SOCSO/PERKESO with payment by the 15th of every contribution month.
  2. For example, the Form 8A for the payroll month of March 2020 must be submitted by 15 April 2020. In this case, April 2020 is the contribution month. 
  3. Submission can be made via i-Assist portal (online) or counter.
  4. This form is generated monthly.
